Emisil vs. RX Sleeve: Which Realistic Penis Sleeve Is Right for ED?
Emisil and RX Sleeve both make hand-finished silicone sleeves that a man wears over his own penis so he can have intercourse when he cannot get or keep an erection. Side by side in a search result they look like the same product at about the same price. They were built for different people. Emisil started in Kaunas, Lithuania in 2007 making gender-affirming prosthetics for trans men, and has since extended that work into sleeves marketed for erectile dysfunction. We started RX Sleeve in California in 2004 to solve one problem, which is how a man penetrates his partner when his own erection will not hold. About 80% of the men we serve are over 60.

We make one of the two products here, so we will tell you up front how we are judging them, and we will say plainly where Emisil is the better buy. A sleeve either works while you are moving or it doesn't. So we compare the things that decide it: how the rigidity is produced, whether the sleeve stays where you put it, how closely it can be matched to your own measurements, what the silicone is and what testing sits behind it, what happens when the fit is wrong, and what buying from another country changes.

Where Each Company Came From
Emisil built its business on prosthetics for trans men, and that shaped what the company is good at. Its catalog grew around three things: soft packers worn under clothing for a natural outline, stand-to-pee devices with an internal funnel, and pack-and-play models with an insertable core so one prosthetic can be worn all day and used during sex. That work demands hand-painting, sculpting, and color matching, and Emisil has close to two decades of practice at them. The ED sleeve is a newer product built on that foundation.
Our founder, Dan Tomlinson, came from a different job. He designed and cast anatomical surgical displays for teaching hospitals before he made his first sleeve in 2004. That work was about getting a physical object dimensionally correct and making it hold up to handling, and it's the reason our sleeves are built the way they are: sized in graduated increments, poured in two firmness levels, and anchored to the body rather than to the clothing.
That history explains most of the differences between the two products. A prosthetic worn under jeans is judged on how it looks and how comfortable it is over eight hours. A sleeve used during intercourse is judged on whether it holds up under force.
How Each Sleeve Stays Rigid During Sex
Rigidity is where most sleeves fail. When a sleeve is worn over a penis that's soft, pushing in sends force back down the length of the sleeve, and at the base there's nothing firm underneath to absorb it. The sleeve folds or pivots down against the lower abdomen. Researchers who study non-surgical ED aids call this the hinge effect. They describe it in vacuum pump erections, where the penis bends downward at the base instead of staying straight, and they note that it gets in the way during sex. The same thing happens to a sleeve that isn't held down at the base.
Emisil handles this by casting the sleeve in layers and offering a semi-rigid rod that slides into a channel inside it. The rod braces the middle of the shaft against bending, and that helps. What it doesn't do is hold the base of the sleeve against your body, so the pivot point at the base is still there.
We handle rigidity two ways. The first is the material itself. Our Firm silicone is poured to hold its own column strength, which is why we recommend it for men who cannot maintain an erection at all, and our Soft option is for men who still get partially hard and want more natural give. You can read how the two firmness options differ before you choose. The second is how the sleeve is anchored at the base, because a stiff sleeve that swings at the base still won't work.
Two accessories exist for men who need more support inside the sleeve. The Adjustable Inner Rod at $40 fills the air gap between the head of your penis and the closed end of the sleeve, so you're pushing the sleeve rather than pushing air. The Girth Stiffener at $25 is a firm silicone tube that goes inside a Soft sleeve, for men who want the softer outer feel with more structure underneath it.
How Each Sleeve Stays in Place
The second failure mode is the sleeve sliding off during use. Every withdrawal stroke pulls on it, and over a few minutes that repeated pull is what moves a sleeve out of position.
Emisil's system holds the sleeve with a silicone base holder and wide elastic straps that snap closed, available in three harness sizes. Elastic stretches under repeated load, which is what elastic is made to do. The give that makes a strap comfortable is the same give that lets the base drift away from the body while you're moving.

The Grip™ is our answer, and it comes installed on every sleeve we make. It has two anchor points instead of one. A soft silicone loop sits at the base, behind the testicles, and that loop connects to an adjustable braided paracord belt about 60 inches long. The paracord does not stretch. When you pull against it, the tension goes into your pelvis rather than into the strap, so the base of the sleeve stays flat against you through both the forward and the backward stroke. It holds the sleeve in place without squeezing the shaft, which matters, because a tight base ring is the usual cause of numbness and cold skin with other devices. The Grip™ is patented, and it's the piece of our design we hear about most from customers.
Sizing: How Each Company Fits the Sleeve to You
Fear of ordering the wrong size is the single most common thing men write to us about before they buy.
Emisil's extension sleeve is made to one internal cavity of about 5.1 inches of internal girth and 5.5 to 5.7 inches of internal depth, with the harness offered in three sizes. That's a workable fit for a range of men, and a simple decision to make.
We size the opposite way. Our girth holes run from 3.75 inches to 6.0 inches in graduated steps, and every model comes in Soft or Firm and in three skin tones. The point of the range is that the inner girth should sit slightly smaller than your natural girth, close enough to hold securely without pinching. Two accessories close the remaining gaps. The Girth Enhancer Kit at $20 is four rollable silicone tubes that build up your own girth so you can wear a sleeve with a larger hole. The Ring Kit at $30 is a set of fit and retention rings. They are not vibrating rings, and they are not constriction rings.
If none of the 24 standard models is right, we build custom sleeves to your specifications through a clay-model review process.
You don't have to work your measurements out alone. Send us your length and girth and we'll tell you which model we would put you in, and our guide to choosing a sleeve walks through the measurements we ask for. Men who talk to us first almost never end up with the wrong size.

What the Silicone Is and How It Is Tested
Both companies use platinum-cured silicone, and that's worth knowing, because most of the cheap sleeves sold on open marketplaces are not made from it. Those are usually TPE or TPR, which are softened with oils and plasticizers and have a microscopically porous surface. Porous means fluids soak in and cannot be fully washed out. It also means they wear out. Generic sleeves in the $50 to $100 range get replaced every few months, while a well-made silicone sleeve in the $200 to $700 range lasts years with normal care.
Platinum-cured silicone behaves differently. It cures without leaving chemical byproducts behind, it's non-porous so nothing absorbs into it, it contains no phthalates or mineral oils, and you can sanitize it in boiling water. On this point Emisil and RX Sleeve are the same, and we'll say so.
Our sleeves are handmade in California from platinum-cured, medical-grade* silicone that an independent lab has certified as skin safe to OECD TG 439, the standard laboratory test for skin irritation. That test uses reconstructed human skin tissue and measures whether the material harms it. It's the same family of testing used for medical device biocompatibility, and it's why we can tell a man the material is safe for him and for his partner rather than just telling him it feels nice. We go into more detail on silicone and skin safety if you want the longer version.
How Realistic Does Each One Look and Feel?
Both companies compete on realism, so here is what each one actually does.
Emisil hand-paints every piece, and the surface detail and base-tone variation in their work are as good as anything in the world. We sculpt ours instead of painting them. Every model is cast by hand in California from a sculpted master, with a shaped head and raised vein detail carried through the mold, so the detail sits in the silicone rather than on top of it. Platinum silicone holds body heat, so the sleeve warms to skin temperature during use instead of staying cool the way a firmer novelty material does. The three skin tones are Caramel, Coco, and Vanilla, and you pick the one that matches you.
Proportion is the part of realism that gets overlooked. A sleeve can be beautifully made and still sit two inches longer than the rest of you, and your partner will notice. Having 24 models across a range of lengths and girths means you can choose one built close to your own dimensions, so it looks right on you and not just on a product page.
What FDA Listing Means When You Buy a Penis Sleeve
External devices that create or maintain enough rigidity for intercourse are regulated. The FDA classifies them as Class II devices under product code LKY, and the FDA's special controls guidance for the category sets out what manufacturers are expected to address, including device design, materials, and labeling about safe use.
RX Sleeve is FDA-listed as a Class II External Penile Rigidity Device. Listing means we have registered our US manufacturing establishment with the FDA and formally declared this device under its product code, which puts us inside the agency's quality system and adverse-event reporting requirements. It is not the same thing as FDA approval, and we do not describe it that way. Approval is a separate and much heavier process that applies to higher-risk Class III devices, such as surgical penile implants.

A foreign manufacturer selling a regulated device into the United States has to register as a foreign establishment, list the device, and name a US agent to handle FDA communication. You can check any manufacturer yourself, ours included, in the FDA registration and listing database. It takes about a minute, and it's the only way to know for sure. If you're comparing brands in this category, we would rather you look it up than take anyone's word for it, including ours.
Buying From Lithuania vs. Buying From California
An Emisil order ships from Lithuania and is priced in euros, so your bank's conversion rate and any card foreign-transaction fee are part of what you pay. Parcels usually clear customs in a few days. Shipments declared as regulated medical devices can also be held for examination, which is a risk that doesn't exist on a domestic order.
What matters more is what happens after it arrives, because this is a several-hundred-dollar item that has to fit your body. Emisil's published policy treats its prosthetics as handmade, specially commissioned, personalized goods. Under EU consumer rules that is the category which sits outside the standard 14-day right to cancel, so a made-to-order piece is not a straightforward return. If something is wrong with it, the process starts with an email and photographs, with a measuring tape visible in the shot when the problem is a dimension, and their terms allow up to two weeks to review the claim. None of that is unreasonable for a small studio making things by hand. It is a slower loop, run by email, across an ocean, under a consumer law that is not the one you live under.
Buying from us, you're buying from a US manufacturer. Prices are in dollars, domestic shipping is $25, and there's no customs step. When something isn't right, you reach a person in California by phone or email during business hours, and the conversation is about repairing, resizing, or replacing the sleeve. We've been doing this for 20 years, and most of the men we help are in their 60s, 70s, and 80s. For that buyer, being able to call someone and get a straight answer matters.
Can You Use FSA or HSA Money?
Often, yes, and this is a real difference between a domestic listed device and an imported one.
The IRS defines a deductible medical expense in Publication 502 as an amount paid to diagnose, treat, or mitigate a condition, or to affect a structure or function of the body. Erectile dysfunction is a diagnosable medical condition, so a device bought to restore the ability to have intercourse can qualify. Benefit administrators won't take that on faith, though. You'll generally need a Letter of Medical Necessity from your doctor naming your ICD-10 diagnosis and recommending an external penile rigidity device.
The administrator then has to verify the purchase, and that's where the manufacturer's status comes in. An imported device with no FDA listing usually can't be verified in the databases they check, and those claims get denied, leaving you to pay with post-tax money. Our US establishment registration and active Class II listing are exactly what they look for. Our page on FSA and HSA eligibility explains how to put the paperwork together, and we'll supply documentation if your administrator asks for it.
When Emisil Is the Better Choice
There are men we are not the right company for.
If you're a trans man, buy from Emisil. Their sleeves, harnesses, and sizing were designed around trans masculine anatomy, and ours were not. The same applies after phalloplasty. A neophallus built from a forearm or thigh flap has a broader, more uniform shape than the anatomy our sleeves are cast for, and Emisil's wide internal bore and wide-band harness are made for reconstructed anatomy and donor-site scarring. We don't design for it and we don't market to it.
If you want one prosthetic that packs under clothing all day and also works during sex, Emisil's pack-and-play line does that, and we don't make anything comparable.
And if hand-painted cosmetic detail is the thing that matters most to you, Emisil's work is among the best in the world at it. For some men, especially those whose confidence has taken a hard hit from ED, how the prosthetic looks affects whether they can relax enough to enjoy sex at all. That's a legitimate reason to choose them.
Which One Should You Buy?
The short version, by situation:
-
You have ED and you want penetration that holds up while you move. RX Sleeve. The graduated sizing, the Firm material, and the non-elastic anchoring of The Grip™ are all built around that one job.
-
You are a trans man, or you have had phalloplasty. Emisil. That's what they build for.
-
You want one device for daily wear and for sex. Emisil.
-
You want to pay with FSA or HSA funds. RX Sleeve, with a Letter of Medical Necessity from your doctor.
-
You are between sizes, or your girth differs a lot from base to head. RX Sleeve, and talk to us before you order so we can size you.
-
You want a US company you can call when the fit is wrong. RX Sleeve.
If you already use a vacuum pump and it gets you partway there, you don't have to choose between the pump and a sleeve. A number of our customers pump first and then put on a Soft sleeve, using the pump for what natural firmness it gives them and the sleeve for the rest.
Do You Have to Be Erect to Use a Penis Sleeve?
No. Our Firm material is made specifically for men who cannot maintain an erection, and it provides the structure on its own. If you get partially hard, Soft is usually the better feel. A firm sleeve does not depend on your erection to work, which is the whole reason it exists.
Will My Partner Be Able to Tell?
Your partner will know you're wearing it, since you'll have talked about it first. What partners tell us is that the silicone warms to body temperature and the sculpted surface reads as skin rather than as a device, and that what they notice most is that sex is happening again. Sensation on your side varies with your own condition, blood flow, and any surgery you've had. Many men feel stimulation through the sleeve and some feel less, and we'd rather you hear that from us now than find it out later.
What Happens if I Order the Wrong Size?
Contact us and we'll work it out, which usually means resizing or exchanging. It's also the situation we try hardest to prevent, which is why we'd rather size you before you order than after.
Talk to Us Before You Order
If you've read this far, you're probably close to buying something, and we would rather you buy the right thing than the thing that ranked first. Send us your length and girth measurements, tell us what your situation is, whether that's post-prostatectomy, diabetes, Peyronie's, or pills that stopped working, and we'll tell you which of our sleeves we would put you in. If the answer is that another company is a better fit for you, we'll say that too. You can reach us through our contact page or by phone during business hours, Pacific time.
